| As an online referral service it is our goal to take care of our patients in the professional manner you deserve. Any personal information shared with us is kept confidential. Our staff treat everyone with courtesy, dignity and respect. Your information will be used by the doctor to offer the best solution for your needs. We treat you as a person not a number. |
| 2. Which doctor and pharmacy do you use? |
| Phoneconsultation.com contracts with board-certified doctors with over 35 years of experience. Your order will be called into your local pharmacy or the hardcopy script will be handed to you at your office visit, you simply go to your local pharmacy and pick up your order. |
| 3. When you order. |
| Please read our terms and conditions page first. When you order with us you are verifying that you are over 25 years of age, are currently using no other service or doctor and have read and understood the terms and conditions. When you have completed the order form, You will receive an automated confirmation email. |

| We Service AL, AZ, CA, CT, DE, GA, IN, IL, KY ,LA, MD, NC,NJ, NM, NV, NY, MN, OH,OK, OR, PA, RI, SC,TN, TX, UT, VA, VT,WI and WV We do not sell, export or import medication nor are we licensed to do so. |
| 5. How much? |

An office visit consultation costs $280, a Phoneconsultation is $250. Both include 2 monthly refills. You pay for your medication at your pharmacy.

| 6. How long from consult to delivery? |
|
Once you place your order with us you will need to send your money order and fax your medical records. We will call you to verify your information and schedule an appointment after we receive your payment. The doctor will then call you to arrange your consultation. Your order will be called into your pharmacy or auto faxed, usually the same day, but expect between 24 to 48 hours. If your script is mailed, we use Regular mail.

| 7. How can you contact us? |
|
Our customer care representatives also help process orders. They can provide information relating to your order only. They cannot answer medical related questions. They can help you through the order process. Please read through the FAQ`s before emailing as most of your questions will be answered. Under no circumstances should you contact the doctors directly. This puts our doctors in breach of our contract, causes them to reschedule clients appointments and increases their daily workload. An unscheduled call to the doctor is $50 and will be added to your consultation fee.

| 8. Do you need medical records? |
|
We require recent records that clearly detail your condition. We need to see the date, the examiner and the examiners contact number. Records can be faxed to us on 1775 593 0631 or uploaded to our secure server from your computer. They are stored in your account for use by the doctor. We also require a clear copy of your valid government issue photo ID.
All orders must have an up to date physical exam form filled out and Current ID!

| 9. What is your Refund Policy? |
| If for any reason you are not satisfied or you would just like to cancel your order, please contact us within 2 days. We will try to correct any problem or send a refund. Please contact our customer service representative at your nearest customer service number. |
| 10. How do i order ? |
|
When you are ready to order just click the order button at the top of the page. You will be asked to create a user account. Once you have created your account, fax us your medical records and mail a payment for the consultation fee.
Office Visit Consultations are available at present , AL, CA, CO, GA, FL,IN, OH, PA and WI. Once customer services receives your payment they will arrange an initial telephone call from the doctor, who will arrange an appointment for your office visit.
Note: Most of our doctors can also see customers in neighboring states.
RETURNING CUSOMERS: If you are a returning customer and you know your medical records and ID are up to date, or you have had your office visit, simply mail your payment a week before you are due a new consultation, as soon as we receive it we will schedule you immediately. Most of our doctors will resume telephone consultations once you have had an initial office visit.
Once you have seen the doctor you will know his address, phone and email, Please do not call the doctor direct, for any problems or a re-consult. Our staff are there to answer any questions or problems you might have, and to reschedule you a Re-consultation. It is protocol to always go through phoneconsultation, our doctors will redirect you or be in breach of their contract..

| 11. Why did I not receive my order or a consultation? |
There are several reasons your order might be delayed or declined. Some of these suggestions may or may not apply.
We have not received your payment. The required information is incomplete or inaccurate. We have not received records. You may have an unfulfilled order with a different company. Your request is too soon. You are registered as a double dipper or doctor shopper. You have already had an office visit with our doctor through another site. You are registered as a frequent flyer.(excessive complaints that orders are lost, stolen, wrong or undelivered) It is Sat or Sun or a Federal Holiday. |

| 12. Why am I being charged an additional $50 or more. |
|
When talking to the doctor this is your chance to have your questions answered. All calls made to the doctor unscheduled will be charged $50 per call. Your Order will be held until this fee is paid. We recommend that you write down your questions prior to your call.

| 13. How can I get a Refund |
| Please browse the website and understand what we offer before you order. If you have spoken to the doctor and been prescribed medication we cannot issue a refund. If you cancel your order before your consultation and your details have not been sent to the doctor, if the doctor cannot help you, or if you clicked multiple times and have multiple charges we will gladly issue a refund. If you have received more than 1 order confirmation this does not mean you have been charged more than once. Double dippers, drug seekers and doctor shoppers will not be issued a refund. |

| 14. Can I pay online |
| Yes. If you pay your bills online throuigh your bank, simply make us one of your bills. Your bank may use www.popmoney.com Or you can go into any wells fargo and deposit your payment into our account. you can also transfer funds from your bank to ours online. |
| 15. I need another consultation, before my next refill. Please direct me how to do so. |

Log in and click on `order` and fax us a copy of you money order for $140 for the interim consultation. You cannot receive the same medication until your prescription is exhausted.

| 20. What If there is a Stop payment, Returned check ,Late Fee ,Bank fee involved in you order? |

there will be a $35.00 charge.
Note: A late fee will be charged on any payment not received one month or 30 days after your consultation, or if a Faxed Payment does not arrive.

21. When will a shipping fee be charged
Some customers will be mailed the hard copy prescription by FedEx, we charge $30 for this service.
